Therapy Fees   

Unless another agreement has been made prior to or during our work together, or I am contracted with your insurance company to accept their fees, fees are the following:

Initial evaluation - $200  

45-50 minute session - $150

60 minute session - $175  

I am currently an In-network provider for the following insurance panels, which means I am contracted to accept their payments as full payment for medically necessary and covered services in addition to any copayment, coinsurance, or deductible due by you at time of service.  I will bill your insurance.

In-network: Aetna, Regence Blue Cross Blue Shield of Oregon, Cigna, Lifewise, Moda(ODS), Optum, Pacific Source, Providence, United Behavioral Health, Value Options.

If I am out of network with your insurance company, you will need to pay the full amount above.  Most insurance will refund you a certain amount, typically 60-70%.  As a courtesy, in most cases, I can do this billing for you.
